You Are Not Crazy. Your Doctor Just Wasn't Trained.

A manifesto on why VULVAi exists, and how the healthcare system needs to change.

If you have pelvic pain, recurring UTIs, or symptoms no one can name, here’s the truth nobody tells you - it is probably not in your head, it is in the system.

As a board certified urologist and sexual health specialist, Dr. Maria Uloko has spent years watching patients suffer for something entirely preventable. Their conditions weren’t rare or untreatable, the doctors in front of them just were never taught how to diagnose them.

A Gap Built Into Medical Training

Dr. Uloko trained first in men's health, before completing a fellowship in comprehensive sexual health for all genders. She is one of only seven physicians trained in this specific model of care. That number alone should alarm you.

There is currently no standardized curriculum for gynecologic, vaginal, or pelvic care that integrates urology, gynecology, pelvic floor therapy, and colorectal medicine. Training depends entirely on where a doctor happens to study. Compare that to cardiology, where every doctor learns the chambers of the heart. Nobody would accept a cardiologist who skipped that. Yet in pelvic health, skipping entire diagnostic frameworks is treated as normal.

This is not an accident. It is a design failure, and patients pay the price.

The Same Story, Told Thousands of Times

Dr. Uloko has heard it over and over. Years spent chasing a diagnosis. Being dismissed. Being gaslit. Getting hope, then losing it. Being told nothing is wrong while something clearly is.

Most of these conditions are curable. Not just manageable. C U R A B L E. The problem was never the disease. The problem was that no one had the training to find it.

When a doctor has never learned about a condition, they can’t imagine it exists. So when a patient describes real symptoms, the response is disbelief. The patient then internalizes that disbelief, and starts to think maybe it really is just them.

It is not just you. It was never just you.

Why the System Stays Broken

This is a public health crisis sitting in plain sight. Billions are spent every year treating chronic and recurrent vaginal and pelvic conditions that could often be fixed permanently with the right diagnosis. Instead, the system spends more money treating symptoms while the underlying cause goes unnamed.

Some point to a lack of research as the excuse, but research requires funding, and funding decisions are choices. Underfunding is not evidence that a field lacks importance. It is evidence of what institutions have decided is worth prioritizing. Calling something "chronic" or "recurrent" often just means it was misdiagnosed, not that it cannot be solved.

The system has quietly benefited from this ignorance. If doctors do not know a curriculum gap exists, they do not have to fix it. If patients do not know their symptoms have a name, they stop asking questions. Everyone stays stuck.

Advocacy Should Not Require a Medical Degree

Dr. Uloko has spent her career teaching patients to walk into appointments prepared, informed, and confident enough to say clearly: “this is my body, and we are making decisions together”. This should not require insider knowledge of the healthcare system, it should be standard.

Patients deserve every treatment option, knowledge of the risks and benefits of each, and the power to choose based on their own life, not a physician's unspoken bias. Shared decision making only works when patients walk in with real information, not fragments pieced together from unreliable search results and well meaning but unverified online communities.

What Needs to Change

Two things have to happen at once - Doctors need standardized training across the entire pelvic health system, not fragmented specialties working in isolation. And patients need direct access to accurate, personalized information the moment symptoms start, not after years of being dismissed.

Technology can close that gap right now, while the slower work of reforming medical education catches up. Personalized insight based on real symptoms. Clear language to bring into an appointment. A way to walk in informed instead of walking in hoping to be believed.
